Skin Anarchy

In this episode of Skin Anarchy, host Dr. Ekta Yadav sits down with Giovanna McCarthy, founder of PDO Max, to explore the history, science, and artistry behind one of the most transformative tools in modern aesthetics — PDO threads. With over four decades of experience in skincare and medical aesthetics, McCarthy shares her unparalleled perspective on how the industry evolved from early European skincare traditions to today’s advanced, minimally invasive procedures.

From Burn Care to Beauty Innovation

Giovanna’s journey into aesthetics began with a deeply personal story — her mother suffered severe burns as a child, sparking a lifelong fascination with skin healing and plastic surgery. Growing up in a European household where skincare was considered a cultural practice, not a luxury, she was exposed early to modalities like mesotherapy and laser rejuvenation, long before they entered the U.S. market.

In 1990, she opened one of the first day spas in America, eventually evolving it into a medical spa as the field became more regulated. “There wasn’t even a ‘medical aesthetics’ industry back then,” she recalls. “We had to build it from the ground up.”

The Rise of Threads — and the Art of Subtlety

When McCarthy founded PDO Max in 2014, she recognized the power of polydioxanone (PDO) threads to restore skin structure and quality without invasive surgery. Unlike permanent PLLA threads, PDO threads dissolve naturally within a year, triggering collagen and elastin production, hydration, and subtle tissue tightening.

“What people miss,” she explains, “is that threads aren’t meant to replacesurgery — they’re meant to support it.” Done correctly, threads act as an internal scaffold, restoring volume and repositioning fat pads that weaken with age.

McCarthy emphasizes that results depend on education and precision. “You can’t learn this in a class of 30 people. Proper training means understanding anatomy — the fascial planes, the subdermal tissue — and working one-on-one until your technique is exact.”

Beyond Lifting: The Cellular Science

At a cellular level, PDO threads create micro-injuries that stimulate a natural healing cascade — boosting microvascularity, collagen synthesis, and dermal hydration. “They literally bring the skin back to life,” says McCarthy. “They improve turgor — that bounce and thickness that makes skin look youthful.”

She warns, however, that over-treating the skin with aggressive energy-based or ablative procedures can have the opposite effect. “Too much too fast makes the skin look like vinyl. We need to stop breaking it down and start rebuilding it intelligently.”

Reinventing Post-Procedure Care

PDO Max has since expanded beyond threads into regenerative skincare innovations such as PDO AfterGlo and LúmEnvy, post-procedure formulas infused with PDRN (polydeoxyribonucleotide) derived from wild salmon DNA. PDRN, long used in burn medicine, enhances revascularization and cellular repair, reducing downtime after lasers, microneedling, or other resurfacing treatments.

“What’s remarkable,” McCarthy explains, “is that you don’t have to inject PDRN to get results. Apply it to disrupted skin and it accelerates healing — that’s what we’ve seen in burn units for years.”

A Future Built on Collaboration and Care

Throughout the episode, McCarthy stresses that the best aesthetic outcomes happen when surgeons, injectors, and estheticians work together — bridging the gap between surgical precision and skin health maintenance.

“Threads, lasers, skincare — they’re all tools,” she says. “But what matters most is education, collaboration, and passion. The goal isn’t to make people look different. It’s to make them look better — healthier, more confident, and more themselves.”
